# Document Uploads

> Upload external documents to create a note, optionally extracting content with AI.

## Upload a Medical Document

Upload external documents (images, PDFs, Word files) to create a note. The AI can optionally analyze the uploaded content and extract clinical information into a structured note.

  <Step title="Start an Upload Note">
    Click **New Note** and select the upload option. Select the patient if not already chosen.
  </Step>

  <Step title="Select a Template">
    Choose a note template from the dropdown. A template is required before uploading.
  </Step>

  <Step title="Upload Files">
    Drag and drop files into the upload area, or click **Select File** to browse. You can upload multiple files at once. Supported formats:

    * **Images:** JPEG, PNG, HEIC/HEIF (up to 10 MB each; large images are automatically compressed)
    * **Documents:** PDF, DOCX (up to 10 MB each)

    You can also skip the file browser and capture a page live:

    * On desktop with the [Scanner Bridge](/guides/scanner-bridge) installed, click **Scan** to pull a page straight from a connected document scanner into the upload list.
    * On a phone or tablet, click **Scan with Camera** to open the device camera and take the photo, which is added to the upload list the same way as a browsed file.

  <Step title="Choose Processing Mode">
    * **AI Processing (default):** The AI analyzes your uploads and extracts medical information (medications, dosages, vital signs, clinical notes) into the template's structured fields.
    * **Skip AI Processing:** Check the **Skip AI processing** checkbox to attach files to the note without any AI analysis. Files are stored as-is.

  <Step title="Generate or Upload">
    Click **Generate Note** (with AI) or **Upload Files** (without AI). After processing, you are redirected to the generated note where you can review and edit the extracted content.

## Source Documents

When a note is generated from uploaded files (via the Upload flow), the original source documents are displayed at the top of the note with a distinct banner. These are the files the AI analyzed to produce the note content.
