# Treatment Plans

> Turn a completed note into a patient-friendly treatment plan, then email it or share it to the patient portal.

## Treatment Plans

A treatment plan is a tidy, patient-facing document you build from a note you already wrote.
You can edit the wording, add your own text, and then send it to the patient by email or
share it to their patient portal. The clinic logo and details appear at the top so it looks
like it came from your practice.

  Treatment plans are available to staff who can edit notes. Only the medications from this
  note are carried over — doses from other notes are left out.

### Create a Plan from a Note

1. Open the note's **actions menu** (three-dot icon) and choose **Generate treatment plan**.
2. If you have saved templates, you can instead pick one under **From template** to start
   with your reusable layout, already filled in from the note.
3. The plan opens in a full-page editor, pre-filled from the note.

### Edit the Plan

* Type directly in the document to change wording, like any text editor.
* Use **Add from note** to pull in more sections or individual questions from the note.
* Type **@** to insert patient, provider, or clinic details, or a link.
* Use **Insert placeholder** to add fields such as the patient's name or a note answer.
  Placeholders are highlighted; hover over one to see what it fills in with. They stay in
  place so the layout can be reused, and they turn into real values when the plan is sent.

### Send or Share the Plan

From the **actions menu** in the editor:

* **Send email…** — review how the plan will look, then email it to the patient as a PDF.
* **Add to patient portal** — publish the plan so the patient can read it when they log in.

Every time you email or share a plan, it's recorded under **Delivery history** in the same
menu, so you always have a record of what was sent and when.

### Reuse a Plan as a Template

1. Set up a plan the way you'd like to reuse it, using placeholders for the parts that change
   per patient.
2. In the **actions menu**, choose **Save as template** and give it a name.
3. Next time, pick that template under **From template** when generating a plan, and the
   placeholders fill in from the new note automatically.

### Where Patients See It

When you add a plan to the portal, patients find it under **Treatment Plans** after they log
in, shown with your clinic's branding. Emailed plans arrive as a PDF attachment.
