Prerequisites: Configure custom measurement fields and groups before recording measurements that are not already set up.
Measurements track patient values over time, such as vitals, body composition, lab results, range of motion, or assessment scores.

Where measurements appear

Open a patient chart to view measurements in two places:
  • Overview — the Measurements section shows the latest values in each group.
  • Medical Notes & Data — the Measurements section gives more room for tables, charts, goals, and editing.
If a value is recorded for a field that is not in a group, it appears under Other Measurements. If Show Measurements is enabled in Settings > Patient Portal, patients can also see measurements in the patient portal.

Record measurements from a patient chart

Open the patient chart

Open the patient and go to Overview or Medical Notes & Data.

Start a measurement entry

In Measurements, click Add.

Choose what to record

In Add Measurements, select a measurement group or open Individual Fields and choose one field.

Enter the measurement date

Set Measurement Date to the date the values were taken.

Enter values

Fill in the fields you want to record. Required fields are marked with an asterisk.

Save

Click Save. The values appear in Measurements on the patient chart.
If a field already has a value on the selected date, Decoda shows Last: with the existing value. Leave Keep existing selected to avoid adding another value, or switch to Record new to save a new entry. Each measurement group can be viewed as Table, Cards, or Chart. The available view depends on how the group was configured in Settings > Measurements. For numeric fields, use Set Goal or Edit Goal to track a target. Goals can appear on cards, tables, and charts. Calculated goals may appear automatically when Decoda can calculate them from related field goals. To change a recorded value, use Edit Value on that entry. Calculated values cannot be edited directly; update the values they are based on instead.

Values from forms and notes

Measurements can also come from forms and clinical notes:
  • A form that includes Measurements or Measurement Groups saves those answers to the patient chart when the patient submits the form.
  • A note template that includes Measurements or Measurement Groups saves those answers to the patient chart when the provider clicks Save.
  • An AI Scribe note can fill measurement fields from the visit conversation. Review the values before clicking Save.
These values appear in the same Measurements sections as values entered directly from the patient chart.

What can be recorded

Measurement fields can collect:
  • Numbers, such as weight, pulse, or A1C.
  • Text, such as a clinical score label.
  • One choice from a fixed list.
  • Multiple values from a list or typed in by staff.
Calculated fields, such as BMI or BMR, are filled in by Decoda after the needed values exist.
