Treatment plan templates standardize the plans providers send to patients. Write the wording once, attach it to a note template, and every plan generated from that kind of note starts from the same text with the patient’s own details filled in. Templates are created here and nowhere else — a provider can’t save a one-off plan as a template from a note. Once a template exists, providers reach it from the note’s actions menu, either automatically or by picking it under From template. See Treatment Plans.

Create a Template

Open the page

Go to Settings > Treatment Plan Templates and click New Template.

Name it and pick a note template

Enter a Template name. Then choose which note template it applies to. When a provider generates a plan from a note built on that template, this plan template is applied automatically. Leave it on No note template if you’d rather providers pick the template by hand.

Write the plan

Type the plan text as you want patients to read it. Use Insert placeholder for anything that changes per patient.

Save

Click Save.

Placeholders

Insert placeholder drops a field into the text that fills in when the plan is generated. The menu offers:
  • Every field on the attached note template — listed by question label, so a plan can pull the provider’s own findings straight out of the note.
  • Patient first name and Patient full name
  • Clinic name and Clinic phone
  • Patient portal link and Payment link
Note fields only appear once you’ve attached a note template, and the list changes if you switch to a different one.

Manage Templates

The table lists each template with its Name, the note template it Applies to, and when it was Created. Click any row to edit it. To remove a template, open it and click Delete — plans already sent to patients are not affected.
Permissions: View Treatment Plan Templates lets a user open this page. Manage Treatment Plan Templates is required to create, edit, or delete. See Roles and Permissions.
